side-effects are common, occurring in at least 10% of patients. These include anorexia, nausea, abdominal pain and diarrhoea. Initial reports with phenformin showed a high incidence of lactic acidosis with an event rate of 40–64 per 100 000 patient years.13 Phenformin was removed from the US market because of the risk of lactic acidosis in 1977. The incidence of lactic acidosis with metformin is markedly lower than with phenformin, with two recent meta-analyses showing no evidence of an increased risk of lactic acidosis associated with the use of metformin compared with non-metformin therapies.
